If you would like to apply to adopt a Charlieoodles puppy please complete the application below. Once your application has been approved you will need to pay a deposit within 12 hours to secure the chosen puppy (or position on the Priority list)

Are you home owners? If no please tell us a little about your living situation ie how long have you been renting for and how long you expect to continue renting? Do you have written permission from your real estate/landlord to keep a puppy
Will somebody usually be home during the day with puppy ? If you have answered yes approximately how long would puppy be left alone for during the day.

Do you have young children at home? If yes please list ages below
Do you ever intend to ever breed from this puppy? Please note we are not against breeding however not all puppies will be suitable for breeding so we will need to discuss this with you further if these are your intentions
